I agree to participate in four quarterly online surveys over the next 12 months and understand I will be compensated for each survey as follows:
$10 for timely completion of the first survey (June 2008),
$10 for timely completion of the second survey (September 2008)
$15 for timely completion of the third survey (December 2008)
$20 for timely completion of the fourth survey (March 2009)
$10 bonus gift card if I sign up by May 23, 2008
Registration form must be received by May 23, 2008 to earn early sign-up $10 gift card.
I understand I will have 2 weeks to complete each survey; if I do not complete a survey, I will not be eligible for the gift certificate reward and will not be eligible to complete any more surveys on this panel.
